What is Community Connect?

Community Connect is a no-cost, community-based referral and support program. Our Community-Based Care Coordinators help individuals and families connect with local services such as housing, healthcare, behavioral health, food, transportation, and more. Services are open to anyone in need—regardless of income, insurance, or background.

How Can We Help?

• One-on-one case management
• Connection to local and statewide services
• Help applying for public benefits and assistance programs
• Support setting and working toward personal goals
• Ongoing follow-up and care coordination

Available Services Include:

Childcare, clothing, dental care, food, housing, insurance, medications, mental health, primary care, employment, education, eldercare, utilities, transportation, substance use treatment, vision care, safety planning, and more.
