Primary Accountability
This position is responsible for coordination of implementation, outreach, and navigation efforts for the Recovery Navigator Program in Grant County. The person will work collectively with the Regional Recovery Coach Manager and operations leadership to supervise Recovery Navigation services in Grant County. This position will supervise Recovery Outreach Specialists who provide field-based case management for people experiencing Substance Use Disorder (SUD).
Primary Duties and Responsibilities
1) Provides onboarding, orientation, training, and mentorship to Recovery Outreach Specialists employed
by Advance and community partner agencies.
2) Supervises Recovery Outreach Specialists in their daily activities, ensuring compliance with program
guidelines and standards.
3) Oversees the preparation of program-specific intake summaries, treatment plans, and case summaries;
ensures ongoing maintenance of confidential records, charting of members’ treatment or care plans and
progress.
4) Acts as the point of contact for law enforcement, community agency partners, and others making referrals to
Advance’s Recovery Coach Services as directed.
5) Through collaborative services agreement and in accordance with community partner agency fully executed
contracts, provides direct services (field-based case management outreach coordination, overdose prevention
education, etc.) to recovery coaching program participants as needed:
a. Responds to community referrals and interacts with individuals who might need case management
or ongoing referrals to external services
b. Performs intake and needs assessments
c. Facilitates warm hand off to supportive services
d. Follows up with and attempts re-engagement of program participants in the community when there
is indication of disengagement
6) Coordinates with case management staff, supervisors, and EMS allies to ensure immediate access and
response to individuals identified as needing services.
7) Responsible for collection, compilation, and preparation of narrative and data-based reports as necessary or
all grants supporting Grant County’s team and community.
8) Provides consultation to other care providers and health educators on matters relating to SUD treatment
modalities, field-based case management for people with SUD and criminal legal system involvement,
overdose prevention, and increasing access to care for people impacted by SUD.
9) Contributes to teams at county and regional levels to establish and maintain productive
partnerships and strengthen relationships between various stakeholders:
a. County level service teams in assigned counties
b. County level service teams with community partner agencies, with particular focus on law
enforcement agencies.
10) Responsible for:
a. Scheduling of staff including, but not limited to call schedules for 24/7 warm handoff response.
b. Coordination of after-hours on-call phone response to Law Enforcement and other RNP partners
referring arrest diversions to RNP case management; this may occasionally require in-person
response to the EMS station or other facility staffed by 24/7 reception (in-person response will be
based on staffing and safety factors).
c. Monitoring supply of program-specific materials including medication (i.e. naloxone) and outreach
supplies, monitors medications for outdates, determines community/county need, coordinates
ordering and performs routine inventory of medication and supplies in collaboration with local site
staff.
11) Assists with assurance of fidelity to program contracts and related documents (i.e. social care network
documents, funding contracts, RNP Uniform Standards.
12) Will work to the level of affiliate licensure or certifications (i.e. CPS or Recovery Coach Certification) to
maximize their contribution to collaborative community case management in a team based harm reduction
model..
13) Program-specific documentation:
a. Relays and/or oversees relaying of appropriate information between agencies for recovery coaching
clients and their care teams by monitoring use and implantation of ROIs.
b. Monitors and processes voicemail, email, direct messaging, text messaging, and/or EHR messaging
in a timely manner.
14) Engage in other Supervisor roles as needed and directed.

Professional and Technical Knowledge
1. Job duties require knowledge and training in areas related to substance use disorders typically acquired
through a formal Bachelor level degree program in Chemical Dependency, Counseling, Sociology,
Social Work, or Psychology, or equivalent combination of lived experience of severe use disorder/s and
incarceration plus non-academic education and previous work experience.
2. For applicants without a Bachelor level degree, acceptable equivalent combination of lived experience
and/or non-academic education includes:
a. Personal experience of substance use disorder/s
b. Personal experience of criminal legal system involvement and incarceration
c. Coursework completion within the comprehensive SUD treatment pathway including Intensive
Outpatient Treatment, Relapse Prevention, and Moral Recognition Therapy or the equivalent
d. Successful completion of requirements and graduation from the Drug Court program or multi-
year probation/parole system or equivalent experience; case-by-case exceptions may be
considered.
3. Equivalent education or experience working with clients with moderate to severe substance use
disorder/s; familiarity with screening, assessment, and evidence-based psychosocial treatments for SUD.
4. Equivalent education or experience with evidence-based therapeutic interventions for SUD (e.g.,
Contingency Management) and brief, structured counseling techniques (e.g., motivational interviewing,
behavioral activation).
5. Willingness and demonstrated education or experience (on volunteer or paid basis) working with
underserved, transient and/or populations at high-risk for morbidity or mortality related to substance use.
6. Ability to effectively document services in the client record using an Electronic Community Case
Management system and hard copy/paper documentation system.
7. Ability to create, send and manage email.
8. Ability to access and use web-based applications and other computer programs including telehealth
modalities.
9. Ability to facilitate additional clinic- or community-based services for teens and adults within the patient
population, if directed and with appropriate training, such as group-based communication/mutual
support and harm reduction services.
Licenses and Certifications
