Community Health Worker - Non Emergency Transportation Driver

Primary Accountability
This position is responsible for non-emergent transportation of supplies or individuals between two predetermined locations. This person will likely drive a van and will occasionally pull a small trailer of supplies to community events. This person will spend a large amount of time transporting individuals to and from medical/BH/SUD appointments, to and from pharmacies and any other service that increases their access to care or eliminates a barrier to preventing the spread of COVID-19 infections. This individual may also provide basic health information to the public as scripted by Advance’s leadership team. They will also assist patients with connections to health, social and other applicable services. This position will also maintain a good driving record, following all laws set forth for operation of a motor vehicle by Washington State’s Department of Licensing. This individual must maintain safety as a top priority for the passengers, themselves and the public around themselves at all times.
Qualifications
- At least 21 years of age
- Valid Washington State Driver’s License
- Good driving record
- Empathetic and kind attitude towards all individuals
- Must pass a background check and pre-employment drug screen.
Primary Duties and Responsibilities
1) The driver will utilize a vehicle supplied by Advance for any and all transportation of clients.
2) The driver will operate the vehicle in a safe manner, obey all traffic laws and adhere to safety guidelines.
3) Maintain communication with the office, specifically the transportation coordinator as needed.
4) The driver will transport clients to and from predetermined locations as instructed by the transportation coordinator at Advance.
5) Responsible for completing drive log (notebook) in a complete and legible manner before and after each trip.
6) Responsible for keeping the vehicle clean and fueled for daily use. Fuel for the vehicle will be purchased strictly with company issued credit cards designated for this purpose.
7) Responsible for respectful communication with clients, co-workers, community partners and all other encounters while representing Advance.
8) Assists clients with accessing community resources (via phone, assists with appointments, transportation to and from appointments, etc.).
9) Establishes community and patient contacts through outreach activities.
10) Reports the following to Advance leadership:

12) The employee will attend community events where Advance is represented. This may include events that occur on the weekend or after regular business hours.

- a. vehicle maintenance needs (i.e. mileage reached for oil change, fluid replacement needs, etc.)
- b. minor vehicle damage (i.e. rock chips in windshield)
- c. spills or cleaning needs inside the vehicle
- d. outside wash needs (i.e. when having traveled in muddy terrain or wet/dirty road conditions)

14) Attendance:
- a. Ensures attendance and hours worked are accurately recorded in our computerized timekeeping system.
- b. Properly manages any paid time off (vacation or sick leave) or unavailable time, communicating this to leadership at least two weeks in advance.
- c. Work hours are flexible and may include some evenings and weekends.

- Must have experience or ability to understand working with underserved, transient, low-income populations and those affected by poverty and other social issues.
- Must have experience or ability to learn popular education methods and other strategies to effectively provide health education to varied community populations.
- Ability to prepare basic correspondence and simple reports using a computer.
- Ability to create, send and manage email.
- Ability to use computers to create tables and simple displays of information.

Work Environment
Work is performed in a variety of settings: primarily inside of a vehicle, may enter and exit the vehicle often to notify clients of their arrival, may also work at indoor and outdoor spaces within the community and field based settings determined by the time and place of the call received for outreach services, community events at indoor and outdoor spaces, may also spend time at a desk in an office location.
Field-based settings will occasionally include homeless encampments and/or other outdoor areas frequented by unhoused individuals as well as institutional settings including shelters, clinics, hospitals, community organization buildings and county jails. Field-based work settings will be vetted for safety in advance and will not occur without accompaniment by other Advance Personnel, but there is inherent exposure to risk in these settings that differs from office-based work.
Training in safety protocols will precede exposure. Any and all institutional visits, if they occur, will be subject to the safety protocols of the specific institutions. Position is based out of Okanogan Advance Office Building, and will require travel to other Advance sites and community locations as determined by the transportation coordinator. Must have reliable transportation and provide documentation of auto insurance and valid driver’s license as required.
Ability to flex schedule when necessary to accommodate special program needs and community events. Street outreach requires the ability to easily navigate city streets on foot and tolerate a variety of weather conditions with unrestricted physical mobility (i.e. walking for up to 3 miles).
